Ex-Voto pieces are traditionally found on the walls of Catholic churches as offerings to request a favor, or to thank a specific saint believed to have the power to intercede in miraculous ways to save a life or a limb.  The arm and the leg in this piece are modeled after Barbie, speaking of unattainable ideal aesthetics steeped in the cult of the body.
